I found an open source EMR called openEMR. I wrote to get more information about 2 weeks ago, but have not heard back. I called the contact number but the phone is not working.
Perhaps this project is not currently active.
I went ahead and downloaded the software. Appears to be written in PHP.

Immunization Connect
A question I always get is: "what do you know about (vendor name)." There is a great need for a starting place to find out about what is going on across the nation when it comes to immunization interfaces. The current fragmentation of effort has meant that immunization registries benefit very little from work done in one location.

2008 Vendor Roundup
I am currently conducting the 2008 Vendor Roundup, to find out what support software vendors have for immunizations. I have identified nearly 700 vendors and I am contacting them in random order. Not all vendors are interested in participating but those who do seem to have a very good support for vaccinations and registries.

openemr is active, but you have to spend some time finding the community. I recommend Rod Roark of sunsetsystems. I would also look at MirrorMed/ClearHealth (which I work on), OpenMRS and VistA.

I took a look at sunsetsystems and this seems to be a good source for information about openEMR. I think the next step is to download the software and review it. Thank you ftrotter for providing this information!
I think there are probably quite a few wanting to look into the OpenEMR, so thanks for the information! I've read a few more articles about it since you posted this so there is probably more to find on it now.
